Basically the gist of this is that I have been pondering over nursing notes, more so in relation to works, which is after my last rather long shift. A patient with significant mental health issues was admitted to our care, and it was very hard to finish the documentation that as well was efficient. At certain instances, it gets too overwhelming that it feels like whatever I scribble on paper, the basic information will still be lacking. Have any of you come across something workable in the mental health notes structure, or its just the way it is doing and undoing?

So, I've been thinking about nursing reports lately, especially after my last shift. We had a patient with complex mental health needs, and it was challenging to document everything clearly while also being efficient. Sometimes it feels like no matter how much I write, I'm still missing something essential. Have any of you ever found a good way to structure mental health notes, or is it just trial and error?

We are having issue with our GIFS showing a black box with what appears to be a menu icon on our GIFS when viewed on mobile devices.
We have paid for the subscription to remove watermarks etc and wanted to see if we were doing something wrong with the setup of our GIFS.
